** As of July 15, 2011, the forms posted on this web site reflect the forms that are approved for use by the Chief Administrative Judge pursuant to V.R.R.P. 84, as recently amended by the Vermont Supreme Court. A process to revise and update the forms previously incorporated in the Appendix of Forms for the Rules of Probate Procedure is in progress. As new and/or revised forms are approved by the Chief Administrative Judge, they will be posted on this web site and the old forms that they replace will be removed. Please use the posted forms until updated versions are available.
