Death Certificate Information


Errors contained in Death Certificates can be corrected by filing a Petition with the Probate Court

Click here to download a copy of the Petition for Correction of a Death Certificate from the above link. Forms are also available at the Probate Court.

The completed and signed Petition must be accompanied by a certified copy of the death certificate and evidence regarding the correction(s) that you wish to be made. Acceptable evidence includes: Birth Certificate, Funeral Records, Medical Records, Police Reports, and/or any other official documents showing the corrections needed. Whenever possible please file certified copies (with official signature and raised seal) of all evidentiary documents.

The Petition may be mailed or personally delivered to the Probate Court. If you are mailing the Petition, please include the filing fee of $25.00.  Please keep a copy of the Petition for your records.
