Rule 80.6. Change of Name
(a) Adults. An adult who wishes to change his or her name may file a petition with the probate court. The petition shall substantially conform to the approved form contained in the Appendix of Forms.
(b) Minors. A parent or guardian of a minor who wishes to change the name of the minor may file a petition with the probate court. The petition shall substantially conform to the approved form contained in the Appendix of Forms. Consent of a parent or a guardian to a minor's change of name may be obtained pursuant to Rule 16. A parent or guardian who has not consented to the change of name shall be notified pursuant to Rule 4 of the commencement of the proceedings.
(c) Venue. The petition shall be filed in the probate court of the district of residence of the person whose name is sought to be changed.
(d) Notice of changed name. Except when the change of name is in connection with the adoption of a minor, the probate court shall cause notice to be published pursuant to Rule 4(e) of each change of name which it has authorized.
